ICC is a service dedicated to assisting youth and families with the highest levels of mental health needs, so that they can live in their homes and communities and realize their dreams. ICC is based on the model and principles established by the National Wraparound Initiative (NWIC) and is an evidenced-based practice that is firmly grounded in a system of quality care values. The standard duration for this program does not exceed 14 months with the hope of youth/family progress and lessening the intensity of support.
In this model of care, a care coordinator is the point of contact for the youth/family and is joined on the team by a variety of people, including family members, coaches, service providers, stakeholders, school personnel, etc. This team is committed to creating and carrying out a plan of care that focuses on meeting the needs of the entire family, not just the young person referred. The overall goal of ICC is to ensure that families realize their hopes and dreams, and experience success in their homes, communities, and schools. This intensive and creative team approach makes ICC service delivery unique.
There is no additional cost for ICC/Wraparound services. ICC/Wraparound is not a treatment provider, a crisis response service, or a funding stream/source, but rather a modality of quality, evidence-based care.
Eligibility Criteria
- D.C. resident
- Medicaid recipient or Medicaid eligible
- Children and youth ages 5-21
- Involved in two or more public agencies
- Deemed at risk of placement outside the home and/or community
- Returning to home or community from acute care, residential and/or psychiatric treatment.
Starting the Process
At the beginning of the ICC/Wraparound process, your care coordinator will:
- Contact you to schedule a time to meet and discuss concerns and issues that led you to join this ICC model of care.
- Request to hear from all members of your family or whoever the family wants to be present during this meeting.
- Make sure any urgent needs are addressed and leave the meeting with a crisis plan and supports to get to the first Wraparound team meeting (WTM).
- Listen to your family’s story to learn about the items that are most important to your family, the people who support you, services and supports that have been helpful in the past, and how your life would be different if you had the resources to meet your family
- Introduce Wraparound to potential team members—including people who are helpful or providing services to your family—and invite them to join the Wraparound team.
What can my family expect?
There are four phases of the ICC/Wraparound service:
Each month a Youth and Family Team Meeting will be held and facilitated by you and the care coordinator. During this monthly team meeting, a discussion centered on goals, needs, updates and successes will be held. The hope is that families will be open to trying new things, taking on tasks, reaching out to others for help and support, planning for crises/emergencies, and using their team to meet family goals. Care coordinators will make visits to your home more frequently in the early phases of ICC/Wraparound services. A key piece of the monthly meetings is flexibility; they can be held at your house, your child's school, our office, or another safe place of your choosing. Decisions about treatment, needs, services, and supports are made during the monthly meetings through collaboration, discussion, and planning.
