Integrated Care Coordination (ICC)
Integrated Care Coordination (ICC) is a service dedicated to assisting youth and families with the highest levels of mental health needs so they can live successfully in their homes and communities and realize their dreams.
ICC is based on the principles of the National Wraparound Initiative (NWIC) and is an evidence-based practice grounded in quality care values. The standard duration does not exceed 24 months, with the goal of reducing intensity of support as progress is made.
A Care Coordinator serves as the primary point of contact and works alongside family members, providers, schools, and stakeholders to create a plan of care that supports the entire family.
Eligibility Criteria
- District of Columbia resident
- Medicaid recipient or eligible
- Ages 5–21
- Involved in two or more public agencies
- At risk of out-of-home placement
- Returning from residential or psychiatric care
What to Expect
Families participate in monthly Youth & Family Team Meetings (YFTM) facilitated by the Care Coordinator. These meetings focus on goals, needs, progress, and crisis planning.
- Meetings can be held at home, school, office, or another safe space
- Families are encouraged to try new strategies
- Plans are developed collaboratively
